Home Hospice Services

East End Hospice develops an individualized plan of care for every patient in consultation with the patient's primary care provider and EEH's Medical Director.

The plan of care may include:

-Medications and equipment, such as oxygen and hospital beds, delivered with no co-pays.

-Personal care, such as bathing, dressing and feeding, performed by hospice aides.

-Visits by nurses and nurse practitioners, who are available 24/7 to educate caregivers about the day-to-day needs of their loved one.

-Emotional support and help with practical matters, like insurance benefits and final arrangements, from licensed social workers.

- Visits from volunteers to the offer socialization caregiver respite, light housekeeping and shopping. Speech counseling, physical and occupational therapy evaluations, massage therapy, Healing Touch, art therapy, pet therapy, spiritual guidance and pastoral care.

Opened in March 2016, East End Hospice's inpatient facility, the Kanas Center for Hospice Care, accommodates patients whose pain or symptoms cannot be managed at home or in other settings.

The Kanas Center features:

-Eight patients suites each with private facilities, ample storage and outdoor space.

-Round-the-clock skilled clinical staffing under the directions of East End Hospice's Medical Director.

- A 24-hour visitation policy that accommodates family, guests and pets.

- Public areas that feature a curated collection of artwork and are designed for family and guests, including a Great Room, library and kitchenette.

- A six-acre campus bordered by the Aspatuck Creek that features native plantings, gardens and walking paths.

East End Hospice offers grief and bereavement counseling to anyone in the community, including families of our Hospice patients.

Services may include:

- Outreach from licensed professionals and volunteers for up to one year after the death of a loved one who received hospice care.

- A growing menu of therapeutic modalities, including pet, equine, art and music therapy.

-Weekly group counseling sessions held at various locations across the East End.

-Camp Good Grief, a week-long day camp for children, ages 4-17, who have experienced the loss of a loved one.

-Individual sessions at our Good Grief Family Bereavement Center in Westhampton Beach, NY.
