Patients in low-income communities often have the greatest health care needs. When the health workers who serve them lack proper resources, it results in negative health outcomes which creates a cycle of increasing burden on providers and patients. Sostento interventions break this negative cycle with uplifting impact.

Project Finish Line

Project Finish Line is a national initiative to mobilize funding, educational resources and digital tools and services to 100 free and charitable clinics in underserved neighborhoods, across 15 states. The goal is to ensure equitable access to the COVID-19 vaccine and boosters and to provide underserved communities with tools and resources to help fight disinformation and the spread of COVID-19. Of note, at the end of 2021, the program received a $7.2 million grant from the U.S. Department of Health and Human Services (HHS), Health and Services Administration (HRSA) to expand workforce outreach in service of vaccine distribution and access.

HelplineSOS is a nonprofit technology platform that assists clinics to rapidly respond to callers needing non-emergency help and information related to COVID-19.

How it works is that people calling into a busy clinic about coronavirus get forwarded to the HelplineSOS automated system which prompts the caller to use their keypad to answer questions related to age, symptoms and if they have underlying health conditions. Answers are populated into a spreadsheet in real time for clinic staff and trained volunteers to make callbacks and provide information, reassurance and routing to services.

Population(s) Served

The Sostento team was given a choice at the beginning of its $7.2 million HRSA program - use the government’s Survey Monkey tool or build our own data collection tool and submit weekly updates directly to HRSA.

Over a two-month period, a team from Sostento built a clinic-friendly data collection tool from scratch and created a prototype data dashboard that allowed for daily checks on the uptake of the data collection tool and then the overall performance of the clinic in COVID services. The prototype data dashboard became a powerful tool in tracking clinic activities and communicating to HRSA and other partners the data collection, network training and management capabilities that Sostento uniquely offers.

Population(s) Served

In 2020, Sostento partnered with Uber Health and the Local Initiatives Support Corp (LISC) to provide free Uber rides for patients to receive their COVID-19 vaccines and boosters. Since its inception, the service has provided more than 10,000 free rides to individuals living in marginalized communities nationwide.

In 2023 and 2024, Sostento aims to grow this program, expanding the number of communities the program serves and expanding the ride service to include a greater variety of trips serving patient health.

Sostento aims to reduce health inequities and tackle social determinants of health in marginalized communities nationwide.

Sostento designs and deploys tools, services, and resources that allow frontline healthcare workers to save more lives. Sosteno does this through the support of the charitable and nonprofit health care providers in its collaborative network. Through evaluation and data collection, Sostento identifies unique solutions that work in one place, and look to adapt and spread them throughout the communities it serves.

Sostento touts a diverse team of doers that include designers, technologists, public health experts, communications professionals and more, all working towards the goal of increasing health equity for low-income populations across the country.

Since its inception, Sostento has grown its clinic network of 100 free and charitable clinics across16 states, provided more than 10,000 free health appointment rides for patients in low-income communities, and developed a state-of-the-art data collection dashboard to allow for real-time evaluation of patients' demographics and success markers.

In the coming years, Sostento aims to grow its clinic network nationwide, expanding the number of clinics and patients served. In addition, Sostento aims to address social determinants of health further, breaking down barriers--like transportation--that prevent individuals in marginalized communities from accessing equitable, high-quality healthcare.
