Direct Care Clinics

Lifewell offers supportive and structured services for adults with serious mental illness, substance use/abuse diagnosis in an outpatient clinic setting. Services are designed to support member independence, enhance insight and develop skills which lead to increased self-sufficiency and community integration, and relapse prevention skills. Services are delivered through a flexible, individualized approach provided in the last restrictive environment possible. Services are intended to maximize independence, personal wellness, self-empowerment, skill development, dignity, self-respect and long-term sobriety in a stable and safe environment.
