What are the organization's current programs, how do they measure success, and who do the programs serve?

Adult Residential Program

There are two branches of our residential program; Comprehensive and Supported Living Services (SLS). Our Comprehensive Residential Program offers adults with IDD 24/7 access to skilled nursing in a group home setting. Services include as-needed assistance with meals, hygiene, transportation and/or access to the community, and support with money management and other personal skills. All support encourages maximum independence for the individual. Supported Living Services are provided to adults with IDD who are living in their own homes or with family in the community. Services include personal assistance with daily living and homemaking skills, employment support, professional services, and transportation.

Day Program promotes computer, math, reading, writing, cooking, and life skills in large and small group settings. Whenever possible, we participate in community activities such as concerts, picnics, hikes, library visits, art classes, physical and recreational programs, etc. Day Program is available to eligible adults in Routt and Moffat County.

Vocational Services allow individuals to participate and work within the community for an employer or on a work crew supervised by a Horizons Job Coach. Horizons helps create good matches for the individual and the employer. Vocational Services are for adults age 21 and older and are available throughout our five county service area.

Early Intervention is for infants and toddlers who have a developmental delay or disability. Eligibility is determined by evaluating the child with parental consent to see if there is a delay or disability in: cognitive skills; communication skills; social/emotional skills; physical skills; and/or adaptive skills. Eligible children can receive services from a Speech Therapist, Occupational Therapist, Physical Therapist, or a Developmental Interventionist through their third birthday. Most children meet milestones and catch up before their third birthday. If a child continues to need support, a referral will be made to the school district for preschool services.

Executive Director

Tatum Heath

Tatum became a Routt County Case Manager with Horizons in 2002. Having an older brother with autism, this move resonated with past experience and quickly became an opportunity to better understand the world of people with cognitive disabilities. In 2008, he became the Adult Program Director and in 2019, Tatum was offered the position of Executive Director beginning January 1st, 2020.
