Transition of out Homelessness Program

The purpose of the Transition out of Homelessness program is to help homeless individuals in Winchester, Frederick and Clarke Counties transition into self-sufficiency.
Program participants will work with professionally trained case workers, Transition Support Specialists (TSS), to achieve goals like gaining admittance into a substance abuse program, receiving appropriate healthcare and obtaining permanent identification.
Advising these individuals when they are in a delicate mental state and saying, “You need to do A, B, C to get out of homelessness” can be overwhelming. TSS work one-on-one, having them set “goal number A”, achieve it, and then set “goal number B.”
